Brow Lift vs. Blepharoplasty: Which Should You Choose?

Should you get a brow lift or a blepharoplasty? This is a great question, and it’s something I often encounter with patients who want their eyes to appear more refreshed.

In this article, we’ll walk through the key differences so you can better understand which approach may be right for you.

Let’s Talk About Your Brow Position

As a rule of thumb:

  • Men: The brow typically sits right at the orbital rim (the bony structure around the eye).
  • Women: The brow usually sits slightly above the orbital rim.

Depending on your brow position, you may be a candidate for a brow lift.

If you look in the mirror and gently elevate your brow, you’ll notice that the eyelid skin re-drapes and the eyes appear more open; that’s one of the key indicators that a brow lift may help.

How Your Eye Shape Influences Your Results

Another factor is the structure of your eye socket:

  • If your eyes sit farther back, elevating the brow often allows the eyelid skin to re-drape nicely, meaning you may not need an upper lid blepharoplasty.
  • If your eyes are more forward (prominent), you’ll likely need to address excess upper eyelid skin as well for the best result.

A Simple Guide to Brow Lifts

brow lift targets the area from the eyebrows up into the hairline. Its goal is to elevate and reposition the brow for a more youthful appearance.

There are several techniques, including:

  • Coronal approach: Incisions behind the hairline
  • Hairline incisions: At or just behind the hairline
  • Direct brow incisions: Placed within or just above the brow

The key is choosing a method that keeps incisions as inconspicuous as possible while achieving a natural-looking lift.

What to Expect from Blepharoplasty

Blepharoplasty (eyelid surgery) involves making an incision in the natural crease of the upper eyelid.

During the procedure, the surgeon may remove:

  • Excess skin
  • Muscle
  • Occasionally fat

This helps the eyes look more open, refreshed, and youthful.

Can You Combine These Procedures?

Yes, and oftentimes, that’s the best approach.

Typically:

  1. The brow is addressed first
  2. Then the upper eyelids are evaluated

This is important because repositioning the brow changes how much eyelid skin actually needs to be removed.

How to Choose the Right Procedure for You

Consider a brow lift if:

  • Your brows sit below the orbital rim
  • Your forehead appears very high
  • Your eyes look tired due to brow descent

Consider an upper blepharoplasty if:

  • You have excess upper eyelid skin
  • Your eyes feel heavy
  • There’s visible fat bulging

Consider a lower blepharoplasty if:

  • Your eyes look tired due to fat protrusion under the eyes

Your Recovery Timeline: What to Know

Brow Lift & Upper Blepharoplasty Recovery

  • Sutures are removed in about 7 days
  • Incisions are usually well-camouflaged
  • Some bruising may occur (varies by patient)

Lower Blepharoplasty Recovery

  • Often no visible incision (transconjunctival approach)
  • Temporary pinkness in the lower eyelid skin
  • Makeup can typically be applied after 7–9 days

Patients may also wear glasses during recovery to help conceal healing areas.

Start Your Journey to Brighter, More Youthful Eyes

Understanding the difference between a brow lift and blepharoplasty is the first step in choosing the right approach for your goals. You don’t have to do everything at once, but in many cases, addressing the brow first creates a better foundation for any eyelid procedure.
